Download hard drive wipe form.
IUPUI requires departments to send all pcs and related equipment to our Surplus warehouse.
Departments must send a Surplus Pick-Up Form in order to be scheduled for a pick up. IUPUI requires departments when surplusing CPUs with the hard drive still intact, that they fill out the hard drive wipe form, signed by the Departments LSP. This form must be attached to the computer when it is picked up. For more information regarding Policies and How to Securely Remove Data http://www.itso.iu.edu/howto/secure-delete.epl
If the CPU that the department is sending to surplus has been utilized to store sensitive data, patient data etc. the department should remove the drive prior to sending it to surplus. Removed drives should be stored securely in the department until the department is ready to have Surplus pick them up and take them to a shredder. When a CPU is sent to surplus with the drive removed the department is required to indicate such on the CPU with a notation attached to the unit.
Hard drives disposal is managed by Surplus through a shredding program contract through Dell Asset Recovery Services.
